Current year’s report card (for 1st - 6th Grade students only)
Student Records Release Form
If divorced, the most recent copy of court documents, signed by a judge and notarized, regarding custody and educational decisions. If documents indicate joint conservatorship, please submit a signed, notarized statement from the other parent stating his/her approval of the student attending Fayette Christian School and tuition scale.
A professional report of any documented diagnosis (e.g. psychological, neurological, IEP, 504, speech, allergies, etc.)

Admission Interview and Campus Tour
The admission interview is an opportunity to tour the campus, discuss philosophy compatibility, answer your questions, and assess whether our program meets your needs. Admission interview appointments are scheduled once the application file is complete. Preschool and Elementary students will also go through a pre-admission assessment administered by Fayette Christian School.

ACCEPTANCE
Official acceptance or status is communicated to all families in writing. Only a Contract of Enrollment duly signed by parents and Fayette Christian School guarantees enrollment for your student. Unsigned contracts are null and void after a two-week period following issuance. By signing the Contract of Enrollment, parents and students agree to Fayette Christian Schools policies/procedures and also agree to attend the mandatory Beginning of the Year “Warrior Welcome” Orientation. Any family who removes a student from school after acceptance of the signed contract will be subject to a $500 penalty per student for breaking the enrollment contract.
Financial ASSISTANCE
If your family would like to apply for financial assistance, you may do so through our third party partner, FACTS for a non-refundable $35 fee.
ADMISSIONS POLICY
FCS is an open-enrollment school, and a Tennessee licensed exempt private school. This license assures an environment that meets or exceeds Tennessee Education Standards in all areas including, curriculum, days of instruction, and safety standards, and more. This license is renewed every two years, keeping FCS accountable and current in state standards.
